The triage practitioner should consider the level of concern of the patient/carer as well as the data collected using the assessment tool to decide on the appropriate action to initiate. If the patient’s presenting problem is an acute emergency, such as collapse, airway compromise, severe haemorrhage or severe chest pain, then the assessment process should be shortened, and contact details and essential information collected. Emergency services should be contacted immediately and immediate care should be facilitated.

An advice will show according to clicked answers and 1 Red will trigger an OVERALL ACTION PLAN of urgent medical review, 1 Amber will trigger a review within 24hrs and 2+ Amber will escalate into Red.
